The MH case manager is responsible for case management and referral
planning for clients in the Access Triage Center suffering from
substance use, mental health, or co-occurring disorders. Duties
include, assisting in completing mental health assessments, care
coordination, maintaining positive working relationships with
outside community providers, scheduling appointments, providing
initial transportation from the triage center, assisting with
acquiring medications, ongoing care and discharge planning, and
crisis intervention. The case manager will consistently collaborate
with Program Manager, Forensic Peer Specialists, Nurse Prescriber
and Case Manager to ensure clients basic needs are being met in
preparation for stabilization and discharge planning.

We are seeking a Mental Health Case Manager who is passionate about
serving diverse communities. Bilingual candidates fluent in one or
more of BHSD's threshold languages-Chinese, Spanish, Tagalog, or
Vietnamese-are eligible for an additional incentive.
In this role, you'll make a difference in the lives of people with
substance abuse challenges, and often with co-occurring conditions.
You'll use and grow your skills in compassionate care, crisis
intervention and de-escalation, in a supportive and inclusive work
environment. You'll have the opportunity to provide our community
with substance abuse treatment to help people who are just
discovering possibilities in their journey, going from a place of
hopelessness to a place where they are valued and have the
potential for a better life.
Responsibilities:
Conduct Assessments to determine client's needs for both mental
health and substance use.
Provide case management services to clients at Access Triage
Center.
Schedule and arrange client medical, psychiatric, and or continuing
care appointments.
Maintain appropriate notation and documentation in client file.
Maintain accurate documentation in accordance with case management
billing practices.
Provide timely crisis intervention when needed.
Meet with all clients while at a triage location to determine case
management and referral needs.
Participate in all required consultations with Alameda County and
Behavioral Health providers regarding client care and case
management experience and opportunities.
Maintain positive working relationships with community providers
and managed care organizations.
Assist with case management training at staff meetings.
Provide clients with educational materials and orientation of
continuing care options, including withdrawal management, mental
health services, residential treatment, and recovery
residences.
Demonstrate good decision making on a daily basis.
Utilize email and other forms of technology to communicate with
peers, supervisors, and community members.
Attend all scheduled team meetings and trainings.
Assist with data entry into EHR system as needed.
Drive program vehicle when necessary.
Perform any other duties as assigned by Program Manager.

Experience
Experience working with customers or clients that feel overwhelmed
or in a crisis (crisis intervention).
Experience working with clients who have substance use
disorders.
Extensive experience with case management of underserved
populations in a public setting.
Experience working with diverse populations and cultures.
Experience working with customers or clients with co-occurring
disorders and disabilities.
Familiar with customer service principles.
Experience with resolution of conflict in stressful situations.
QUALIFICATIONS: (education and work experience)
Currently enrolled in Masters level program working towards
licensure and/or registration with the California Board of
Behavioral Sciences (BBS) as an AMFT or ASW.
Experience providing case management services in a mental health or
substance use disorder program in the non-profit sector.
Experience working in a crisis setting.
Preferred: Bilingual in English, Spanish, Chinese, Vietnamese, or
Tagalog
