    • Must have acute hospital experience in CM and discharge planning
    • - Do not submit CM's with only SNF, REHAB or Hospice experience
    • - Must have prior travel experience
    • - Certification is not required.
    • - Must be willing to work in weekend rotation
      • Job Requirements
      • -1 yr. minimum recent experience with discharge planning as a Clinical Case Manager with a heavy focus on discharge planning in an acute care hospital setting
      • - Need to have acute INPATIENT case manager experience.
      • - BLS
      • - InterQual
      • - EPIC
      • - Allscripts/Careport
        • Schedule Information
        • - Must be willing to work in weekend rotation
          • Additional Details
          • - 2 or more years of experience preferred
          • - Utilization Review is a plus
          • - ACM or CCM Certification is a plus
          • Sensitive personal data redacted
